Body Piercing & Permanent Make-Up Terms and Conditions:

The following terms and conditions are concerning the services provided by Beauty Mark LLC/PMU & Piercings.

These terms and conditions must be read in full before scheduling your service. Every client must provide their state issued form of identification, if you are over the age of 18. If you are a client under the age of 18, then you must provide a birth certificate, a school id and or passport, and the parent must provide their state issued ID. These are non-negotiable terms and must be abided to comply with Utah state and the Health Department regulations.

Definitions:

- Artist refers to the piercer or Permanent Make-up artist performing the procedure.

- Client refers to the person receiving the service, whether it be a PMU service, piercing service or a minor receiving the piercing with proper consent from a parent.

- PMU service - Permanent Make-up

Information:

Cancellation of service by the Artist:

- The Artist reserves the right to stop work on a Commission, or cancel a Commission request due to poor communication, rude/inappropriate behavior, or a breach of these Terms of Service.

- I will always inform you of all the information you will need to have a smooth healing process to end with the best results, if at anytime I feel as if you are not going to be responsible for your service I will cancel our service at that time.

Refunds:

Refunds may only be provided under special circumstances and at the artist’s discretion;

- Refund are not common in the “Body Art” world. When you come into your appointment you will be signing a legal waiver of consent for the service with the understanding that all risks and possible long term effects have been discussed. 

- Getting any service done to the body has inherent risk and there for we encourage people to do research on their piercing or PMU service before scheduling. We are always happy to disclose all of the information we believe you will need to heal properly and safely. We want everyone to be happy with their service, so we will always go above and beyond to make sure you the client are satisfied.

-No refund will be provided for clients who do not follow proper after-care procedures and neglect their piercing site. 

Ownership and Copyright:

- Unless otherwise decided, the Artist owns the copyright to any photos or videos of the service being provided. The Client is granted a Personal License to use the images upon full payment of the service. 

Under a Personal License, some things the Client cannot do are: 

- Represent artwork without credited the artist.

- Represent their service or piercing in any way that shows falsehood to the service provided. For example we only use implant grade titanium jewelry for initial piercings, and if you decide to put new jewelry in the piercing before it was healed (and who knows what this new jewelry consists of) and then go online to say you received a “bad service” this would be considered a breach of contract.

Some things the Client can do include:  

- Display artwork/services on social media, as long as representation of the artwork is credited to the artist. No one will expects you to tag us in every photo you post online, just to be give us a little shout out anytime anyone asks you were you got your service done at.

Client Protections:

- Note that all personal information provided to PMUandPiercings is protected and not shared with any other businesses, social media platforms or websites, bedsides for GoDaddy payment processors
